As a parent, keeping your kids healthy is one of the most important things you do. To help with that job, you can request access to your children’s online medical records and services, from birth through age 17, if they get care at a Kaiser Permanente facility.
The first step is to register for your own online services at kp.org/wa or download the Kaiser Permanente Washington app. Then fill out the Request for Parental Access form and take it to the business desk at any Kaiser Permanente medical facility with your photo ID and, if you’re a legal guardian, bring guardianship documents. You’ll receive a message in your kp.org/wa secure message inbox letting you know that you have access.
If you’re not a Kaiser Permanente member and are not receiving care at a Kaiser Permanente facility, you should contact your care provider for instructions.
